نقش میانجی‌گر استرس ادراک شده در رابطه بین سخت‌رویی و کیفیت زندگی در افراد تحت همودیالیز

چکیده
هدف: هدف تعیین اثر میانجی­گری استرس ادراک شده در رابطه بین سخت­رویی و کیفیت زندگی بود. روش: پژوهش توصیفی از نوع همبستگی و جامعه آماری پژوهش شامل تمام بیماران مراجعه کننده 20 تـا 65 سـال تحت درمان با همودیالیز بیمارستان­های امام خمینی، سینا، هاشمی­نژاد، شریعتی، رسول اکرم در بهار سال 1394 بودند. به روش نمونه­گیری در دسترس تعداد 200 نفر انتخاب و به پرسشنامه­های کیفیت زندگی همودیالیز هیزو همکاران (1994)، استرس ادراک شده کوهن و همکاران (1983) و سخت­رویی کوباسا (1984) پاسخ دادند. پس از حذف 3 آزمودنی پرت، داده های 197 آزمودنی با استفاده از روش مدل­یابی معادلات ساختاری تحلیل شد. یافته­ها: نتایج نشان داد مدل ساختاری پژوهش با داده­های گردآوری شده برازش دارد و استرس ادراک شده رابطه بین سخت­رویی و کیفیت زندگی را میانجی­گری می­کند و 33 درصد از واریانس متغیر کیفیت زندگی در افراد تحت همودیالیز به واسطه تغییر در نمره­های ابعاد سخت­رویی و استرس ادراک شده تبیین می­شود.نتیجه­گیری: سخت­رویی در بیماران نه تنها ادراک این افراد از عوامل استرس­زا بلکه کیفیت زندگی آن­ها را تحت تأثیر قرار می­دهد. ارزیابی کیفیـت زندگی بیماران به کادر درمان کمک می­نماید تا روش­هـای درمـانی ارتقـاء کیفیت زندگی بیماران را مورد توجـه قـرار دهنـد.

Aim: The aim of study was to determine the mediation effects of perceived stress in the relationship between hardiness and quality of life of hemodialysis patients. Method: The study was descriptive correlational design and the statistics population of the study included all hemodialysis patients of Imam Khomeini, Sinai, Hashemi-Nejad, Shariati, and Rasol Akram hospitals in the spring of 1394. 200 students were selected by available sampling and responded to quality of life for hemodialysis by Hays et al (1994), perceived stress by Cohen et al (1983), hardiness by Kobasa (1984) questionnaires. After removal of 5 outlier subjects, structural equation modeling was used to analyze the data of 306 subjects. Results: Results showed that research structural model base on gathered data fit well and perceived stress mediates the relationship between hardiness and quality of life and 33% of the variance in quality of life in hemodialysis patients due to changes in perceived stress scores hardiness is explained. Conclusion: Hardiness in patients not only their perception of stress, but it affects the quality of life. Evaluation of the quality of life of patients helps to health care staff to consider the styles of improvement the quality of life of patients.
